This exciting role has been developed to contribute to the more specialist areas of elderly medicine based within OPAL (Older Persons Assessment and Liaison) Services , Nursing Home Service, Orthogeriatric services and Palliative Care at Wythenshawe Hospital. The successful candidate will join the Complex Health team and be based at Wythenshawe Hospital, Southmoor Road, Wythenshawe M23 9LT.

OPAL provides early comprehensive geriatric assessment in both ED and AMU with the aim for timely safe discharge with input from community services to reduce the chance of readmissions and improvement in patients independence. It is an ever growing and expanding service. There will be opportunity for the postholder to rotate through these specialised services and contribute to their development.

OPAL House provides non-acute multi-disciplinary care in a safe environment for those patients who no longer require acute hospital care but who are unable to return home. This maybe as a result of increased frailty after a period of hospital admission, which has resulted from deconditioning or consequence of acute illness.

OPAL House is also able to accept patients from the emergency department who are in crisis and unable to be managed safely in their home environment and therefore may be admitted to hospital but do not have an acute medical problem. This follows the discharge to assess concept which is being adopted across the country as it has been advised that assessment of care needs should not take place in hospital.
